Project Name:Capacity Building Project for the Prevention and Control of Diabetes in St. Vincent and the Grenadines

Project Description:
According to Institute for Health Metrics and Evaluation (IHME) statistics, mortality rate due to diabetes in SVG increased by about 11.6 percent in the past decade (2005 to 2015), with a mortality rate of around 91.24 people/ 100,000 populations.
The purpose of this project is to strengthen the integrated care ability among health workers, promote health knowledge, and provide epidemiological report.
Overall, this project will:
1. Assist in planning effective integrated care strategies and practices on the prevention and control of diabetes.
2. Strengthen the ability of integrated diabetes care in healthcare facilities.
3. Improve health self-management of community people for prevention and control of diabetes.

Project Objectives:
The objectives of this project are:
1. Assist in planning effective integrated care strategies and practices on the prevention and control of diabetes.
A. Health officials to visit Taiwan to understand Taiwan’s policies, integrated care and health education programs related to the prevention and control of diabetes.
B. Health officials to design the SVG integrated diabetes care model with the assistance of consultants from Taiwan, and submit an integrated diabetes care report with recommendations.
C. Hold at least one international conference focusing on prevention and control of diabetes.
2. Strengthen the ability of integrated diabetes care in healthcare facilities.
A. Conduct an intervention study of diabetes health education through utilizing the case tracking and management model, and produce an epidemiology report.
B. Strengthen the function of chronic disease integrated care team in at least one district hospital.
C. Strengthen the hardware equipment which related to NCDs in selected healthcare facilities.
3. Improve health self-management of community people for prevention and control of diabetes.
A. Build a capacity of health education personnel.
B. Hold health education and promotion activities in the community.
Project Performance:
Strengthen diabetes management system in SVG
1. Assist in planning effective integrated care strategies and practices on the prevention and control of diabetes.
(1) Completed four health officials from SVG visited Taiwan and produced a recommendations report on integrated diabetes care.
(2) Conducted an International Conference for Diabetes Prevention and Control.
2. Strengthen the ability of integrated diabetes care in healthcare facilities.
(1) Intervention research proposal of diabetes health education has been approved by the IRB committee of MoH in St. Vincent, and recruited 158 patients.
(2) Completed 19 seed instructors training on diabetes integrated care in Taiwan (including 4 physicians, 10 nurses, and 5 dietitians).
(3) Conducted 10 local trainings on diabetes patient integrated care to 221 local health personnel.
(4) Donated one mobile health education vehicle, five fundoscopy machines, five HbA1C meters, and fifty-five foot care kits to health facilities in St. Vincent.
3. Improve health self-management of community people for prevention and control of diabetes.
(1) Completed 3 seed instructors training on health education and promotion for prevention and control of diabetes in Taiwan.
(2) Conducted 8 local trainings on health education and promotion for prevention and control of diabetes to 94 local health promoters.
(3) Conducted 87 diabetes health education activities, and provided health education to 901 people and HbA1C screen to 1,104 people.
(4) Through local medial to promote the self-prevention on diabetes seven times.
(5) Distributed 400 pieces of diabetes health education leaflets during World Diabetes Day and other health education activities.
(6) Completed filming of 12 health education videos on prevention and control of diabetes.
