Project Name:Capacity Building Project for the Prevention and Control of Metabolic Chronic Diseases in St. Kitts and Nevis

Project Description:

The project will be conducted based on the results of the last project, the Capacity Building Project for the Prevention and Control of Chronic Kidney Disease, and integrate with The Chronic Care Model recommended by WHO and Taiwan’s profound experiences in preventing metabolic diseases. This project will strengthen the metabolic disease care system through assisting medical facilities and personnel in building up early screening capabilities, case management, and shared care between doctors, nurses, and dietitians. Therefore, the project is expected to strengthen their prevention and control system for chronic diseases caused by metabolic syndrome. Overall, this project will:
(1)Assist in planning effective integrated care strategies and practices on prevention and control of metabolic chronic diseases.
(2)Strengthen the care capacity for metabolic chronic diseases at medical institutions.
(3)Enhance the self-management ability of community residents for metabolic syndrome.

Project Objectives:

The objectives of this project are:
1.Assist in planning effective integrated care strategies and practices on prevention and control of metabolic chronic diseases
A.Propose a strategy recommendation report on integrated prevention and control for chronic diseases (mainly diabetes, hypertension and chronic kidney disease) related to metabolic syndrome.
B.Carry out an international seminar on "Prevention and control of metabolic chronic diseases".

2.Strengthen the care capacity for metabolic chronic diseases at medical institutions
A.Strengthen case management of metabolic chronic disease patient information.
B.Strengthen the multidisciplinary care ability of metabolic chronic diseases.

3.Enhance the self-management ability of community residents for metabolic syndrome
A.Improve people's awareness of their own metabolic conditions.
B.Raise public awareness of metabolic chronic disease.

Implementation Arrangement:Members of the Project will provide technical assistance and assistance for capacity building.

Current Progress:

To the end of December 2023, the following activities have been carried out:

  1. Completed "Community Health Screening Report" and "24-Hour Recall Dietary Intake Survey" and provide to Ministry  of Health of SKN for reference. 
  2. Trained 6 seed instructors on metabolic chronic disease care training in cooperation with Taipei Veterans General Hospital in Taiwan. In addition, the training program was switched to the online training program during the pandemic of COVID-19 and 89 medical personnel and 28 health educators completed the course.
  3. On World Kidney Day, a kidney disease prevention and control health education event was held in a high school; and a kidney disease prevention and nutrition workshop was held in Joseph N. France Hospital; On World Hypertension Day, a health education and screening activity have been held in Basseterre Police Station.
  4. Co-produced "Sports Promotion (5 episodes)", "Healthy Diet (26 episodes)" and "Salt Reduction (11 episodes)" with the Ministry of Health of St. Kitts and Nevis. Aforementioned episodes were broadcasted on the National Cable TV Channel, the Facebook and YouTube channels of the Health Promotion Unit (HPU) under the Ministry of Health.
  5. Completed 6 local training session for 65 health personnel.
  6. Assisting the Taiwanese diabetes educator and the Ministry of Health in filming the “In Ya Kitchen” cooking program. By using local ingredients to deliver the message of healthy cooking and disease prevention, and distributed 100 copies of recipes. Coordinated policy on SKN MOVES and cooperated with Health Promotion Unit (Ministry of Health) to publish the first cookbook. Promoting the prevention and control of metabolic chronic diseases health education.
  7. Printed and disseminated 29,550 copies on Hypertension, Diabetes mellitus and Chronic kidney disease health education.
  8. Health education and screening activities have been held in communities, schools, public and private institutions. 334 sessions have been held, and 14,478 people have been screened.
